The only holdup is how to make money and/or charge for content. I doubt people will pay more than say $75.00 per mo for content. That would be for a far greater spread of programming than is currently offered in cable or dish services. that would mean Hollywood would have to improve their product and trim their lifestyle. Everything else is in place now.
